Job Title: Social Worker (LMSW)

Employment Type: Full-Time, Part-Time, or PRN positions available

Job Description:
The Social Worker (LMSW) will provide critical support to patients by assessing their social, emotional, and environmental needs to ensure appropriate care within the hospital and swing bed program. The social worker will conduct comprehensive assessments to establish personalized Plans of Care, collaborating with healthcare providers to ensure patients needs are met. This role involves assisting patients within the hospital setting and facilitating referrals to external resources for further support.

Key Responsibilities:

• Assess patients for appropriateness to the Critical Assess setting, ensuring all criteria are met for hospital and/or swing bed program admission.
• Conduct comprehensive assessments of patients' needs, strengths, goals, life history, and preferences.
• Develop individualized Plans of Care in collaboration with the physician and nursing team to ensure all needs are addressed.
• Work with patients and their families to provide support, guidance, and education regarding care plans and available resources.
• Refer patients to appropriate external services and assist in navigating community resources.
• Document assessments, interviews, and Plans of Care in patient records.
• Ensure that all patients receive the necessary services and resources during their stay.

Qualifications:

• Strong knowledge of medical social work practices, patient care protocols, and available community resources.
• Excellent interpersonal and communication skills, with the ability to work effectively within a multidisciplinary team.
• Compassionate and patient-focused with the ability to provide emotional support to patients and families.
