Join Our Team as a Clinical Liaison!

Position Overview:

Are you ready to make a difference in the lives of patients and their families? We are looking for a passionate Clinical Liaison to join our team at Providence Home Services. This part-time role (28 hours/week) will have you floating between PPMC and PSVMC , where your strong communication and interpersonal skills will shine!

As a Clinical Liaison, you will be the vital link between our home care services and contracted facilities. Your mission? To coordinate and arrange home health, home infusion, and home medical equipment services for patients transitioning from hospital to home. You’ll educate hospital staff about our services and contribute to patient care plans, ensuring a smooth transition for everyone involved.

Your Responsibilities:

• Coordinate home care services between HS and contracted facilities.
• Act as a clinical resource for hospital staff.
• Educate staff on services offered by HS.
• Contribute to patient care and discharge plans.
• Ensure smooth transitions for patients and families post-hospitalization.

What We’re Looking For:

Required Qualifications:

• Oregon Registered Nurse License (upon hire)
• National Provider BLS - American Heart Association (upon hire)
• Criminal background check compliance (upon hire)

Preferred Qualifications:

• Bachelor's Degree in Nursing
• 1 year of full-time experience in home health or hospice
• 1 year of full-time experience in home infusion
• 1 year of full-time experience in discharge planning or care management
• Strong customer service skills in home care settings

Working Conditions:

This role is primarily on-site at our Halsey Bldg-Home Service East Portland location. You will be part of a supportive team dedicated to providing exceptional care.

Pay Range: $50.21 - $70.04 (base pay range; additional compensation may be available)
