  Pinnacle Home Care is currently seeking a PRN Physical Therapist (PT) to work in the Lehigh Acres area!

Position Overview: Physical Therapist (PT)

As a Physical Therapist at Pinnacle Home Care, you’ll play a vital role in delivering high-quality care to patients in the comfort of their homes. You’ll work under the direction of a physician and nursing supervisor, ensuring care meets all regulatory and clinical practice standards.

Key Responsibilities

  • Perform initial and ongoing patient assessments, including OASIS documentation.
  • Develop and implement tailored treatment plans in consultation with physicians, adjusting as needed.
  • Collaborate with the interdisciplinary care team to optimize patient outcomes.
  • Provide education and support to patients and families on therapy programs.
  • Supervise Physical Therapist Assistants every 30 days.
  • Communicate patient progress and changes with physicians and care teams.
  • Document all visits and progress notes on the same day of service.

Qualifications

  • Graduation from an accredited Physical Therapy program.
  • Active PT license in Florida.
  • Current BLS certification.
  • Valid driver’s license and reliable, insured vehicle.
  • At least one year of PT experience (home health experience preferred).
  • Proficiency with electronic medical records is a plus.
  • Physical capability to perform required tasks (standing, bending, lifting).
  • Strong self-management, communication, and judgment skills.
