Frazier Rehab Sports is the official rehab provider for University of Louisville Cardinal Athletics and club sports, Louisville City FC and Racing Louisville FC professional and youth soccer clubs and fourteen local high school athletic programs. Responsibilities: Job Summary The physical therapist is responsible for the evaluation and treatment of patients. In this role you will perform patient evaluation, development and implementation of an appropriate treatment plan, delivery of treatment services, and comprehensive discharge planning. Area of Focus: Sports and Ortho patients Qualifications: Education / Accreditation / Licensure (required & preferred): BS/MS/DPT degree in physical therapy required. DPT degree preferred. Valid PT License Kentucky State Board of Physical Therapy Certification CPR, BLS through the American Heart Association. Experience (required and preferred): Experience in physical therapy preferred for outpatient positions. Acute care physical therapy experience required for acute care positions.
