Palliative Care Physician - Seattle, WA

Job Summary:

We are seeking a compassionate and board-certified Palliative Care Physician to join our interdisciplinary team. This role offers the flexibility to set your own pace with rounds, supported by a dedicated Nurse Practitioner (NP) to assist in managing patient caseloads. You'll provide specialized palliative care to patients in LTAC/STAC and acute care settings, collaborating closely with an interdisciplinary team to offer patient-centered care plans and manage complex symptoms.

Key Responsibilities:
Lead patient care in LTAC/STAC and acute care facilities, supporting clinical decision-making and guiding patients and families through complex medical situations.

Manage serious illness symptoms, including prescribing medications, and monitor treatment effectiveness in collaboration with the NP.

Oversee and direct the development of proactive care plans, ensuring patient safety and optimal outcomes.

Conduct rounds with the support of a Nurse Practitioner (NP) who will assist with patient visits, documentation, and follow-ups.

Collaborate with specialists (e.g., oncologists, cardiologists) and primary care providers to ensure palliative care is integrated into the overall treatment approach.

Facilitate communication between patients, families, and healthcare teams regarding care goals, treatment options, and advance care planning.

Maintain continuity of care across settings, including hospital-to-home transitions, and document all care in our EHR system.

Qualifications:
Doctor of Medicine (M.D.) from an accredited institution

Board Certified or Board Eligible in Palliative Care

Active Washington Medical License

Active DEA License

Proven experience in LTAC/STAC settings preferred

Strong interpersonal and communication skills to connect with patients, families, and healthcare teams

A compassionate, patient-centered approach to care
