Job Title: Home Health Registered Nurse (16 Hour Part-Time)

Overview: Provide in-home healthcare services to patients in the Northbrook, IL area while delivering quality nursing care in a home setting. This role focuses on supporting patient health and independence through skilled nursing services, accurate documentation, and effective coordination with the care team.
Responsibilities:
Provide skilled nursing care to patients in their homes
Perform patient assessments and assist with care planning
Administer medications and treatments as ordered
Educate patients and families on care and disease management
Document visits accurately and in a timely manner
Communicate with physicians and interdisciplinary care teams
Travel to patient homes within the assigned service area
Schedule:
Part-time position averaging 16 hours per week
Flexible scheduling
Travel required within the local territory
Compensation and Benefits:
Estimated hourly rate of $42.00 per hour
Travel reimbursement provided
Comprehensive 6-week RN onboarding program

Qualifications:
Active Registered Nurse license in the state of Illinois
Associate degree or higher in nursing
Minimum of one year of professional RN experience, home health experience preferred
BLS and CPR certification or willingness to obtain
Valid driver's license, reliable transportation, and auto insurance
Ability to travel locally as required
